I, , agree to treat this dog humanely by seeking the help of a veterinarian if the dog should become ill; to obtain annual booster shots for protection against Parvo and Distemper.

I agree to abide by the County's leash law and will never intentionally allow the dog to run free unattended (unless within an enclosed area designated for off-leash dogs).

I agree to make every effort to find the dog if it becomes lost, including frequent visits to local and county shelters (where the dogs can be euthanized after five days holding time).

I agree to inform It’s The Pits of any change of address in or out of state, and if at any time I cannot keep this dog, I will contact It’s The Pits at (858) 472-0985 and arrange to return the dog to It’s The Pits (you will be responsible for transporting the dog safely to us at your expense).
A $1,000 fine will be assessed to you if it is dicovered that the dog has been sold, re-adopted, or given to a shelter, or to any other individual.

I agree to release, hold harmless and forever discharge It’s The Pits, including its volunteers from any and all claims and demands whatsoever arising from this adoption. This agreement supersedes any prior agreement or representations regarding the dog.

I understand that no warranties, either expressed or implied, are made regarding:

The breed of the dog.

The health or medical history of the dog.

Whether the dog has been inoculated for any diseases.

The personality or disposition of the dog.

The appropriateness of the dog for the owner or owner’s family.

This agreement shall be binding on the new owner, the new owner’s family, and all members of the new owner’s household.
